you dont have to loop throught... http://jsfiddle.net/w2PNM/ just get the parent TR tag of current input field and check the children and instead of giving the inputfields names like name_number us...

Why go through all the hassle of using Javascript to dynamically change it to enabled/disabled?? Just add some if-statements to the PHP while-loop where you do the fetch.
